MUNCHKIN RUN WAIVER
By typing my name in the box below, I hereby certify that I am the parent/legal guardian of the children listed below and that the children listed below are healthy, have passed a physical and are capable of running the race distances they are entered in without physical distress.
As the parent/guardian of the children entered into the Mt. SAC Relays Munchkin Run:
- I understand that running is a potentially hazardous activity, which could result in injury or death.
- I acknowledge that they are medically able and properly trained to participate
- I understand that by typing my name below, I assume all risks and liability associated with participating in this event for my children and those under my guardianship which includes, but is not limited to: falls, contact with participants, acts of God (including extreme weather conditions), injuries associated with competition, etc.
- I grant the Mt. SAC Relays the use of photos, videos, recordings or likeness' of my children/those under my guardianship, for legitimate event promotional purposes.
- I acknowledge that there are no refunds in the event of cancellation due to acts of God or participant safety.
By typing my name below, I hereby certify that I have carefully read this waiver and in consideration of my child(ren's), or those under my guardianship, participating in this event, I hereby waive and release the Mt. SAC Relays, Mt. SAC Athletic Special Events, Mt. San Antonio College, all event sponsors, their representatives and successors from all claims or liabilities (of any kind) and waive the ability to bring any legal action against the entities outlined above.
